March 2008: Camper Health History Form Information on this form is gathered to help us in identifying acceptable care for your daughter. To be completed by parents. Fill out and return immediately with appropriate signatures. Please do not hold this form until your daughter’s medical examination.

Have your daughter’s physician fill out this form after her examination and return this to us in PA. Have this returned to us in NH if mailed after June 3rd.

Please share this information with your daughter and return immediately with her signature and the signature of one parent.
