All staff must complete this medical form and return it by mail by June 01, 2008 to:

Government Forms: FORM W-4 / FORM I-9 <BRING TO CAMP COMPLETED>
If you are a citizen of the USA you must complete both the I-9 and W-4 forms and hand them in to the registration table upon your arrival at camp along with a copy of two forms of ID or passport.

If you are not a citizen of the USA, you must only complete the I-9 form and hand it in to the registration table upon your arrival at camp along with a copy of your passport.
